Deadlines

All Forms can be found on FOCUS, or under the Forms and Documents tab. All forms should be sent to Summer Service Coordinator through email, fax, or mail.  Contact information can be found HERE.

June 15th, 2012:

  • Agency Contract: This form should be filled out by the Agency Supervisor. This contract ensures that the student will be able to serve at least 60 hours at their agency.
  • Agency Fact Sheet: The Fact Sheet should state the agency’s history and mission statement, contact information (address and website), goals and programs offered to clients/ people served, the impact the agency has made on the community and the duties you will perform as a volunteer.

Once these forms have been turned in and approved, students may begin their summer service. If you are not able to confirm a placement by June 15th, please contact the Summer Service Coordinator for an extension.

During Your Service

  • (Optional) Journal Entries: Journal entries can indicate why you choose that particular agency, what you learned or discovered during the week, what you did and did not enjoy, any interesting experiences, or preconceived notions that were confirmed or untrue, etc.  Journal entries can also include interviews with agency staff members and/or community members who utilize the services of the organization’s services.
  • Maintain a timesheet that lists daily service hours. You will receive a copy of the timesheet once your assignment has been approved.  You will record your daily hours and your Supervisor will sign each log to confirm hours completed.

August 2012

  • Mayor’s Award: Every year the Volunteer Center of Durham recognizes students who have served more than 100 volunteer hours in Durham community during the summer.  To be eligible for this award, the student must complete at least 100 volunteer hours in Durham (or surrounding areas) by August (exact date yet to be determined). Upon completion of 100 or more volunteer hours, the student will honored by the mayor of Durham, NC for their contributions to the city.  To receive more information about the Mayor’s Award, or to register, contact the Volunteer Center of Durham at 919.688.8977.

August 31, 2012

  • Timesheet and Journals (optional) will be due at the Bryan Front Desk by 5pm.
  • Supervisor Evaluation & Service Confirmation. Without the Supervisor Confirmation, your service requirement is not satisfied.  Please ensure your supervisor submits it on time.
