Transcripts & Records
	An NCSSM transcript includes all your 9th and 10th grade courses, grades, and credits, as well as your 11th and 12th grade NCSSM work and all standardized test scores (PSAT, SAT, ACT, etc.) which were reported to NCSSM during your enrollment. There is no charge for transcripts. To request a copy of your NCSSM Transcript your hand written or typed (email requests are not accepted) request must include: Your full name while enrolled. Your year of graduation. The exact address to which you want the transcript sent *NOTE: To be "official", a transcript must go directly from our office to the school, agency, program, or employer who is requiring it. All transcript sent to students are stamped "ISSUED TO STUDENT". A telephone number at which we can reach you in case of any problem. Your signature and current date. The request should be mailed to: NCSSM Registrar P.O. Box 2418 Durham, NC 27715-2418 NCSSM does not fax transcripts, but we can accept your transcript request via fax, providing it contains all of the above information and your signature and current date.
